Foothills Screening Letter for Parents

Dear Parents,
Foothills Creative Beginnings is pleased to offer in collaboration with Lakeview Community Preschool, classroom wide screenings in the areas of speech and occupational therapies for children who attend a minimum of 9 hours of programing per week.
An Early Learning Coordinator (certificated teacher) as well as a Speech and Language Pathologist and Occupational Therapist will be at the preschool September 2022 to conduct screenings for children in the areas of speech, language and fine motor development who attend school at least 9 hours per week and have a signed consent form.
Please complete the attached screening consent forms and return with your registration package. If you do not wish for your child to participate in screenings, there is no need to complete and return the form. Please note, there is no cost for screenings or services provided by FCB.
Once screenings are complete, parents will receive a screening card indicating if there are areas where your child may benefit from intervention services or if there were no areas of concern.
Should intervention services be recommended, you will receive a call from the ELC to discuss intervention services and the next steps for registering with FCB should you choose to pursue these for your child.

Permission for screening
Your child’s early childhood program has agreed to participate in Foothills Creative Beginnings’ (FCB) Early Intervention Screening to identify possible speech and language, fine or gross motor, regulation concerns. We can also provide support for children who are learning English as a Second Language. We are seeking your permission to screen your son or daughter. This screen should take no more than 10 - 20 minutes to complete and will help determine if Early Intervention Support services provided by FCB may be recommended for your child. Screening usually occurs during programming time at your child’s program site. If your child will not be in attendance on the scheduled screening day, please inform your child’s teacher to make arrangements for an alternate time. If the screening results indicate your child may benefit from Early Intervention services, you will be contacted by one of FCB’s therapists to book an assessment appointment.
Please note, screening. registration and fund application must occur prior to (insert date) for children to receive services funded through Alberta Education.
By submitting this form, I give permission for Foothills Creative Beginnings, to screen my child attending Lakeview Community Preschool.
